Spinal stenosis is a common condition in which the spinal cord or the nerves exiting the spinal cord are constricted. In most patients, it occurs in the neck but it can also affect the lower back, and on very rare occasions it’s been known

Twenty-nine percutaneous balloon dilations of the axillary and subclavian veins were performed in 19 patients. Stenoses occurred in typical locations of anatomic narrowing or at sites of previous trauma. The initial success rate was 76%, with a 1-year patency rate of 35% and a 2-year patency rate of 6%.

Severe stenosis of the proximal subclavian artery causes retrograde flow in the ipsilateral vertebral artery (to bypass the stenosis by collaterals). In partial steal, there is antegrade flow in the vertebral artery in systole, with retrograde flow in diastole. Images of the subclavian and innominate arteries were retrospectively reviewed for evidence of stenosis by an investigator unaware of sonographic results. The percentage of stenosis was calculated by comparing the lumen diameter measured at the point of maximum stenosis to the diameter of a disease-free segment of the subclavian or innominate artery distal to the stenosis. Subclavian artery stenosis is a form of peripheral arterial disease (PAD). It may present with transient episodes of vertigo and circulatory symptoms involving the arms and hands Physical examination may demonstrate a weak pulse in one upper extremity and a systolic blood pressure difference of more than 10 millimeters of mercury (mmHg) between contralateral left and right upper extremities.

The prevalence of subclavian artery (SA) stenosis is approximately 2%. The exact prevalence of extracranial vertebral artery (VA) stenosis is undetermined, with estimates ranging from 7% to 40%. Nearly 25% of ischaemic strokes involve the vertebrobasilar circulation, and arteriosclerotic disease and narrowing of the proximal VA may be the cause for up to one-fifth of these incidents.

2016-09-04 The stenosis was dilated to 8.5 mm, determined by an insufflation table, thereby pushing the surgical clip off the artery caudally. This was seen on fluoroscopy. Leaving the guidewire in place, the balloon was removed and replaced by a 6F monorail catheter (Heart Medical Europe B.V, Best, the Netherlands), which was placed with its tip at the origin of the RSA. Subclavian stenosis, however, is most often asymptomatic and does not require specific therapy, other than that directed at the underlying etiology .

Discussion: Subclavian artery stenosis is an uncommon vascular disease, showing a 4-fold left, rather than right-sided predisposition. Although a low-grade stenosis is usually asymptomatic and may remain unobserved, a severe stenosis may cause retrograde blood flow in the ipsilateral vertebral artery, leading to a medical condition with various clinical symptoms, known as subclavian steal syndrome. Subclavian artery stenosis is a form of peripheral arterial disease (PAD). Although subclavian or innominate artery stenosis is not rare (occurring in 17% of 6,534 cases in the joint study of extracranial arterial occlusion [ 5 ]) flow reversal within the vertebral artery is present in a minority of these cases (2.5% in the same study), and of those with angiographic steal, only 5.3% (9/168) had neurologic symptoms. An ascending brachial venography was requested by the angiology department.

Examination was performed in neutral position (Fig 1) and in upright position (Fig 2) of the left arm. The result was stenosis of left subclavian vein and varicose changes of basilar and cephalic vein of the left arm by outside compression of the left subclavian vein.

In the second coincident right subclavian artery (RSA) stenosis was depicted. In two other cases, the aberrant left subclavian arteries (ALSA) were found to be narrowed at the level of origin by up to 70%. Sometimes that pain in your neck is more than an annoyance.

The lesions treated included short band like stenoses and longer atheromatous segments and were all haemodynamically significant.
